At your first visit, our front desk makes a copy of your insurance card to keep in your file. Please have your insurance card with you at your visit. If you change insurance plans, we’ll update your record at your next visit.
Referrals and Authorization
Certain insurance plans have provisions that require you to obtain referrals when specialty care is needed. You can obtain these referrals from your primary care doctor and, in some cases, a specialty physician. It is best to contact your insurance company to see if your specific plan requires a referral for care. If so, please obtain a referral from the correct source prior to your visit. If you cannot get the referral by your appointment time, we can reschedule you.
The deductible is the covered expense that you and your dependents must pay before your insurance will make a payment. Speak with a representative from your insurance company if you don’t understand your yearly deductible. While we still bill your claims to your insurance company, you’re responsible for covering the payment until you meet your deductible. Keep in mind you effectively do not have a copay until you’ve satisfied your calendar/fiscal year deductible.
